New Client Form

Thank you for choosing Arlington Animal Clinic as your pet’s health care team! We look forward to meeting you and your pet. Please fill out the following new client form and attach your pet’s previous medical history. Once received, a member of our team will reach out to you within 2 business days to schedule an appointment.

Please be advised, due to the unprecedented demand for pet care, we do require a deposit in order to schedule an appointment. This deposit will, in turn, be applied to your first visit. In the event that your needs change, please be aware that we have a strict 24-hour cancellation policy for all appointments. Failure to provide sufficient notice or failure to keep your scheduled appointment will result in the loss of your deposit.
